Responsibilities
* Evaluate and provide feedback to direct care staff (home health aide) in the home per policy.
* Provide education and training to the direct care staff while in the client’s home, if necessary
* Audit client charts as needed to ensure aides are following the Plan of Care and documenting correctly
* Assess clients as needed
* Perform client medication set-ups, on an ongoing basis, as needed
* Provide in-service education topics to RN manager based on deficits of care observed during the supervisory visits or reported from the clients
* Completes all documentation timely and accurately
* Available as a clinical resource to staff, as needed. This requires being available by phone outside normal schedules
* Report any concerns/issues to Branch Manager
* Maintains confidentiality regarding client information and all related documentation per HIPPA guidelines
Requirements
* Licensed RN in the state of Michigan
* At Least 2 years’ experience in acute or long-term care while employed as a RN
* Valid Driver’s License
